New patient guide

Your first visit, made a little simpler.

A little preparation helps keep the appointment focused on what matters: your questions, your health, and the next step in your care.

Before

Confirm the visit

Use the secure scheduling details once the clinic publishes them.

Prepare

Gather the essentials

Bring your identification, insurance card, medications, and records.

At the visit

Build a clear plan

Talk through priorities, questions, and what should happen next.

What to bring

Four things that help us get started.

If an item is not available, do not delay needed care. Bring what you have and let the care team know what may be missing.

  • 01
    Photo ID

    A current government-issued identification card.

  • 02
    Insurance card

Your current card and policyholder information, if applicable.

  • 03
    Medication list

Names, doses, and how often you take each medication or supplement.

  • 04
    Relevant records

Recent test results, vaccine records, and specialist notes related to your care.

What to expect

From check-in to a clear plan.

A first visit establishes a baseline, makes space for current concerns, and identifies the care that matters most right now.

  1. 1
    Review

Confirm your history, medications, allergies, and the reason for the visit.

  2. 2
    Discuss

Share questions, recent changes, and the priorities you want addressed.

  3. 3
    Plan

Review next steps, follow-up, testing, or referral when appropriate.

Before appointments open

The practical details are being finalized.

We are keeping uncertain information off the site until the clinic confirms it, so every published next step is one you can trust.

01

Insurance & payment

Plan details

Accepted insurance, eligibility verification, and self-pay information will be published before appointments open.

02

Secure scheduling

Booking access

A verified phone number and approved scheduling or patient- portal link will replace the prior non-secure request form.

03

Records & forms

Safe transfer

Approved forms and secure medical-record transfer instructions will be added when the clinic’s systems are ready.

New patient questions

A few answers before opening day.

This page will continue to improve as the clinic confirms scheduling, insurance, forms, and patient-portal details.

Is Rising Sun accepting new patients yet?

The practice is opening soon, but appointment booking is not yet available. The verified phone number and secure scheduling link will be added here as soon as the clinic confirms them.

Which insurance plans will be accepted?

The accepted-plan list and self-pay details are still being finalized. Coverage can vary by plan, so patients should also verify that Rising Sun is in network with their insurer before a visit.

How should I send records from another office?

Please do not send medical records through an ordinary website form. Secure transfer instructions and the clinic fax number will be published after they are verified.

Will new-patient forms be available online?

The clinic’s approved forms or patient-portal links will be added before appointments open. This page will be updated when those secure resources are ready.

What should I do for an emergency?

Do not wait for a website response. Call 911 for a medical emergency. The site is not monitored for urgent medical needs.
